Totus Tuus drop-off and pick-up is in the parish hall at 8:45 AM.

Release of Liability:
I understand all reasonable safety precautions will be taken at all times during this event. I understand the possibility of unforeseen hazards and know the inherent possibility of risk. I agree to hold harmless the Diocese of St. Cloud, St. Mary of Mt. Carmel parish, and its leaders from any and all claims arising from or in connection with attending this event. My typed signature below indicates my agreement.

Medical Liability:
I understand that the Diocese of St. Cloud and St. Mary of Mt. Carmel assume no responsibility for accidents which may occur in association with this event. I agree to use our personal insurance to cover any such incidents. I understand that, in the event medical intervention is needed, every attempt will be made to contact the persons listed above. In the event they cannot be reached, I hearby give permission to transport my child/ren to a hospital for emergency medical or surgical treatment.
