I understand all reasonable safety precautions will be taken at all times but know the inherent possibility of risk. I agree to hold harmless the Diocese of St. Cloud, St. Mary of Mt. Carmel parish, and its leaders from any and all claims arising from or in connection with attending Faith Formation classes and connected events at the parish. I understand that the St. Cloud Diocese and St. Mary of Mt. Carmel assume no responsibility for accidents which may occur in association with Religious Education events and classes. I agree to use our personal insurance to cover any such incidents. I understand that, if medical intervention is needed, every attempt will be made to contact the persons listed above. In the event they cannot be reached, I hereby give permission to church personnel to transport my child/ren to a hospital for emergency medical or surgical treatment.
